TO ACCESS THE STARRED (*) DOCUMENTS:

The starred documents are compressed (zipped) files, necessary to make them accessible with Internet Explorer version 8 or earlier.

Except for the PowerPoint presentation, the starred files are document forms intended to be filled out using MS Word and then emailed to starbase.programdirector(at)alaska.gov. If you are unable to fill them out on the computer, please fill them out manually, scan them (or print them to a PDF file), and email them to the above address. If you cannot access a form, contact the Program Director for other solutions.

Once we have received the Class List and Class Demographics, we will set up a 30-minutes-per-class visit to give a program overview to the students, conduct a pre-visit assessment, and pick up the teacher agreement, call signs, chaperone forms, and student permission slips.
